MD Codes™ - The devil is in the detail
For many, many years, injectors the world over were pioneers of a kind. They did their work without any form of template or system for success. There was very little in the way of training, and what training there was consisted of one injector passing his or her knowledge on to others. A doctor would treat that area until the folds softened or went away when a patient complained about nasolabial folds or dark circles under the eyes. The problem, as we now know is, that they were treating the symptom, not the cause.
This approach often resulted in hugely inconsistent results because the methods (or lack thereof) didn’t produce anything close to natural-looking results. Treating individual small areas of the face without respecting the subtleties of facial balance and proportion, more often than not, produces an unflattering ‘puffy’ or ‘over filled’ face. Understandably this gave dermal fillers a terrible reputation and turned many people off the idea altogether.
The way we inject today has changed considerably. While there have been many contributors, it is all largely down to one man.
Dr. Mauricio De Maio
Dr. Mauricio De Maio is a highly talented Brazilian plastic surgeon. Dr. De Maio felt increasingly that some of his patients did not need to go under the knife and have a surgical procedure. He felt that the results could be achieved in many cases using precisely place hyaluronic acid; dermal fillers.
Dr. De Maio devised a system for injecting that could be standardized. His approach dealt not simply with treating symptoms but with learning about how the patient felt and why. Together with his assessment, he used his new process to formulate a plan for his patient/s. This first system was called the 8-Point Lift injection technique. It involved placing dermal filler in eight strategic points around the face to lift sagging skin and restore facial volume.
Dr. De Maio observed that as we age, the structural trifecta of a youthful face – bone, fat and collagen – decline dramatically, resulting in wrinkles, sagging and a sunken appearance. Simply filling in the nasolabial folds didn’t make a face look more attractive or rejuvenated. You see, once we developed a more advanced understanding of how our facial structures change over time. How our face works at the time of the consult allows for a much more sophisticated in our approach to dermal filler.
These early processes devised by Dr De Maio became more and more advanced, later becoming the Md Codes – The MD Codes™ system was developed to increase clinician success rates by reducing variability in the technical aspects of hyaluronic acid (HA) filler treatment and focusing on addressing unfavourable emotional attributes of the face.

Dr Sarah Kennea MD has had the privilege of attending a number of training programs conducted by Dr. De Maio. Her skill and expertise in this area was quickly recognized, and she was fortunate to work with some of Canada’s most esteemed injectors to further her skill and knowledge.
In addition to her years of experience as an Aesthetics Clinician, the integration of the MD Codes into Dr. Kennea’s practice revolutionized how she approached injecting and aesthetics. In 2019, the Allergan Medical Faculty invited seven Doctors from across Canada to form a faculty, and to become direct mentees of Dr. De Maio. Dr. Kennea was one of only two women, and the only physician on the west coast invited to join the faculty, embracing this unique opportunity to work with and learn directly from Dr. De Maio and some of the most accomplished injectors in the world.
All patients at Whistler Medical Aesthetics enjoy an exhaustive consultation process with a highly trained MD Codes consultant, who works next to Dr. Kennea. A treatment plans is customized after this extensive consultation, and Dr. Kennea then fine tunes this plan before treatment. During treatment planning, the appropriate MD Codes are selected using algorithms focused on lessening unfavourable facial attributes (a saggy, tired, sad, or angry look) and enhancing positive attributes (an attractive, younger, more contoured, or feminine [soft] or masculine look). Using a state of the art photographic system, patients take a number of photographs before and after treatment. Why does the MD Codes work & why does the approach differ
To understand how MD codes are used to revitalize the face, it is important to review the consequences of aging.
As we age we experience changes that can be boradly consistent withsome or all of the following:-
- Over the years, our natural expressions like frowning or smiling cause wrinkles to form around the eyes and forehead.
- As we age the wrinkles and folds can become more prominent. Typically around the mouth and nose area.
- Excess skin can appear around the jowls, smile lines and underneath the chin.
- We all have natural fat pads under the skin that give the face shape. When these fat pads shrink and shift it can result in lost fullness around the forehead, eyes, brows, temples and cheeks.
- Changes in the face are often described as emotional attributes – For example they can often give a tired and sad appearance.
- Bone density decreases and the structure of the face becomes more hollow, especially around the eyes, cheeks and chin.
These changes that we all experience to one degree or another can be relentless. This is why we do not approach this subject as beauty or beautification, no this is about correction.
